NJ DOT Physical – Here’s What to Expect
The first thing we’ll do when you come for your DOT test is to get your medical history. You’ll need to supply the following information to do this:
- Fill out the Driver Information and Health History parts of the Medical Examination Report form and get it signed and dated.
- Name and phone number of the doctor who prescribed any current prescriptions that you are now taking
- Medical documents for any significant disorders, such as diabetes or heart disease,
- Please include a letter from your treating physician if you are taking medication to address any of the above diseases, or to help you stop smoking.
Urine, eyesight, hearing and blood pressure will all be tested as part of the physical exam. To be considered medically fit, you must pass each exam according to DOT standards.
Vision Test
To read traffic signs, monitor road conditions, and more, commercial drivers require strong eyesight. Each eye must be able to see at least 20/40, with or without the aid of a corrective lens. On the eye chart, you will be required to read the characters in a specific order. Additionally, the nurse practitioner will check each eye’s peripheral or side vision. For your final test, you’ll be put through a series of tests to see if you can identify between the colors of traffic lights.
Note to drivers: If you wear eyeglasses or contact lenses, bring them along.
Hearing Test
You must be able to hear a forced whisper from at least 5 feet away, whether you use a hearing aid or not, according to the DOT. If you don’t, you’ll need to take an audiometric hearing test. This is to ensure that you will be able to hear the noises of traffic and any radio announcements. Make sure to bring any hearing aids you use to your consultation.
Blood Pressure Test
You must have a blood pressure equal to or less than 140/90 to be eligible for the 24-month certification. A shorter certificate may be issued to those whose blood pressure is higher than 140/90 but less than 180/110.
Take a few effortless precautions to guarantee that your blood pressure test goes smoothly and provides reliable results:
Take a few effortless precautions to guarantee that your blood pressure test goes smoothly and provides reliable results:
- 30 minutes before the test, refrain from drinking coffee or smoking cigarettes.
- Keep your feet flat on the ground and sit with your back supported for five minutes before the test. Rest your arm on a table at heart level.
- It is preferable to have an empty bladder, so go to the restroom first.
- Wear short sleeves.
We may average the values obtained from two measurements taken two minutes apart. Also, if you have any queries, we’ll gladly explain your blood pressure reading in terms you can comprehend.
Update Your Medical Certificate (MCSA-5876)
A Medical Variance granted by FMCSA is required if your DOT Medical Examiner’s Certificate states that you “must be accompanied by a Diabetic, Hearing, Seizure or Vision waiver exemption or a SPE Certificate.”
To keep your commercial driving privileges, the DMV must receive and process your updated Medical Examiner’s Certificate and any relevant Variance before the expiration date of the Medical Examiner’s Certificate on file. A two-week processing time is recommended for a new certificate.

Conditions That Can Disqualify You from Passing Your DOT Physical
Before taking the DOT physical test, drivers should be aware of some of the most frequent health issues that might cause them to be disqualified from the exam.
In addition to epilepsy, alcoholism, the use of habit-forming substances (such as opiates and amphetamines), as well as amputation or impairment of a limb, there are many more conditions that may be considered. If you fail the vision, hearing, or blood pressure tests, you will be deemed ineligible. The nurse practitioner at Preventive Medicine Associates can help you determine if any of these conditions qualify for an exemption.
